For any questions or concerns regarding the privacy policy and practices for the Feedback Evaluation App, please contact the Goodman Surgical Education Center at surgery-goodmancenter@stanford.edu.

2. User Data:

The Feedback Evaluation App will collect user name and email during account creation. User data will not be shared, distributed, or sold outside of internal uses for research purposes, faculty development, and quality improvement.

Automatically Collected Personal Information:

Whenever you visit or interact with our Sites, we may use a variety of technologies that automatically or passively collect information about your online activity. This information may be collected in the following ways:

  • Through your browser: Certain information is collected by most browsers, such as your IP address and Internet browser type and version. Your “IP address” is a number that is automatically assigned to the computer that you are using by your Internet Service Provider (ISP). An IP Address is identified and logged automatically whenever a user visits the Sites, along with the time of the visit and the page(s) that were visited. We use IP addresses for purposes such as calculating Site’s usage levels, helping diagnose server problems, administering the Sites, and presenting content that is tailored to your country.

  • Through Cookies: Cookies allow a web server to transfer data to a computer for recordkeeping and other purposes. We use cookies and other similar technologies to, among other things, better serve you with more tailored information and to facilitate your ongoing access to and use of the Sites. 3. Data Security:

We are committed to protecting your data and ensuring your privacy. Our app includes password protection and other measures to safeguard your information. Here’s how we approach data security:

  • Password Protection: Access to your account is secured by a unique password. We recommend using strong, complex passwords and changing them regularly to enhance security.

  • Data Encryption: While we use basic encryption for data transmission, please be aware that sensitive information should still be handled with caution.


  • Access Control: Only authorized users can access their accounts, and we implement measures to prevent unauthorized access. If you suspect any unauthorized activity, please notify us immediately.


  • User Responsibility: You are responsible for maintaining the confidentiality of your password and for all activities that occur under your account. Please do not share your password with anyone.


4. Data Retention & Deletion:

In the event that use of the Feedback Evaluation App is discontinued, all user submitted data will be retained until completion of related research and educational activities. 

Users may permanently delete their Feedback Evaluation App account at any time by completing the following form: https://airtable.com/appJCJiUrCVSBr0Wv/shrmhqoQEwbRSpvc0 or use the form embedded below. All user submitted data will be permanently deleted with the deletion of the account.
